INSTRUCTIONS ON PREPARING AN ORDER FORM

Please prepare an ORDER FORM for each product item or setof product Items (see list on product items under PRODUCT DESCRIPTION section). This separation is necessary to avoid cluttering order specifications when more than 3 product items are specified.

On CUSTOMER DATA section, complete all information required and make sure details are accurate. This is important for shipping purposes and making further communications should there be unexpected problems processing your orders.

On PRODUCT DESCRIPTION and DESIGN SPECIFICATIONS section, it is highly preferred for faster processing of orders that our company is provided with final artwork on garment design and printworks from which to base final production. However, if this is not readily available, simply provide us with complete written order specifications and/or design references. This will serve as guideline in preparing final artworks to be forwarded to you within 2 to 3 days for approval.

On INDIVIDUAL LISTING section, carefully indicate individual names, numbers, and quantity and use the pull-down menu to indicate product items and sizes.
